Your monthly fee includes primary care visits within our practice. You can also contact your care team between appointments. Membership supports preventive care, sick visits, and chronic condition management.

Some tests, medications, and procedure supplies carry separate charges. We explain those costs before you receive the service.

Membership Details

Midwest Medical offers straightforward membership options for individuals and families. Your monthly fee covers everyday primary care and ongoing access to your care team, with no surprise membership charges. Explore what’s included, understand potential additional costs, and find the option that works best for you.

Individual Memberships

Adult memberships are $95 per month. Children under 18 are $35 per month with an enrolled parent or guardian.

Family Memberships

Family memberships are $240 per month. This includes two adults and up to three children under 18 in the same household.

Included Care

Membership includes office visits, appropriate phone or video visits, and communication with your care team. You can receive routine checkups, sick care, and ongoing condition management.

Additional Costs

Outside laboratory testing, medications, and certain procedure supplies are billed separately. We review any additional charges with you before proceeding.

Health Insurance

Membership pays for primary care within Midwest Medical and isn’t health insurance. Keep insurance for emergencies, hospital care, and specialist services.

Enrollment and Cancellation

Membership is month-to-month, with no enrollment fee. You can cancel with 30 days’ notice before your next billing date.
